Certificates of Insurance

Fill-out the certificate of insurance request form

Please email or fax the supporting documents (certificate request from certificate holder, literature of event, a copy of the completed permit application or contract)

Provide the following information:

      • Certificate Holder name, address, phone #, fax #
      • Certificate requirements (additional insured, type of insurance needed, etc.)
      • What is the activity or event
      • Dates of activity or event
      • Location of activity

Your request should to be submitted at least 2 weeks prior to the event.
